This offer is subject to the following terms & Conditions
1) The £100 Cash Back Offer is open to UK residents aged 18 years or over. Employees (or family members of employees) of Positively Healthcare, its affiliates, subsidiary companies, any representatives or agents of these companies may not enter. Positively Healthcare reserve the right to verify the eligibility of entrants. This promotion is limited to the original intended recipient of the promotional message. Applications from individuals who have not been sent the promotional message by Positively Healthcare are not eligible to participate in the Promotion. This offer is limited to only one claim per qulaifying company or individual. Any cash back payments will be made by cheque. No other form of payment is available.
2) The offer begins 00:00:01 BST on 1st October 2008 and will be reviewed or closed 00:00:01 1st January 2009.
3) To receive a £100 cash back payment, entrants must: (i) have current private medical insurance (ii) Must NOT have healthcare supplied by their bank (ii) Must not be a BUPA direct individual policy holder. (iii) Must complete our Letter Of Authority which will be sent to you upon completion of your contact details or when telephoning our offices. (iv) Prospect must remain a client for a full 12 months following the renewal of their policy.
4) When completing your contact details you must provide Positively Healthcare with a valid email address or telephone number.
5) Positively Healthcare reserve the right to add additional terms and conditions to this offer. Positively Healthcare reserve the right to withdraw this offer at any time without prior notice.
6) Successful Entrants will receive their £100 cashback payment from Positively Healthcare within 60 days of completion of a second appointment.
7) In the event of any circumstances arising outside of Positively Healthcare's control, we reserve the right change or cancel this offer.
8) If Positively Healthcare has suspicions or knowledge that any applicant has been involved in any fraudulent or illegal activity in relation to the office, Positively Healthcare reserve the right to deem the application void.
10) Positively Healthcare shall not be liable, in contract or tort (including negligence) for breach of statutory duty, or in any other way, for any loss or damage howsoever arising suffered by an applicant resulting directly or indirectly from this offer. Nothing in these terms shall limit Positively Healthcare's liability for death, personal injury or fraud.
11) Positively Healthcare does not accept, and is not responsible for, any lost or delayed applications. Positively Healthacre would like to remind applicants of the nature of limitations of the internet and is not responsible for any problems or technical malfunctions of any communications network, online system or computer hardware or software that may affect entry to the offer.
12) Positively Healthcare will use reasonable efforts to ensure that all information relating to the offer is accurate, Positively Healthcare is not responsible for any problems or technical malfunctions of any communications network online system or computer hardware or software that may affect Positively Healthcare operation of the offer or the Positively Healthcare website, including without limitation the accuracy or any offer Material.
13) Positively Healthcare may use third parties to facilitate one or more elements of this Promotion and may provide some of your personal identifiable information to them. These third party service providers are subject to confidentiality agreements with Positively Healthcare and other legal restrictions that prohibit the use of the information Positively Healthcare provides them for any purpose except to facilitate the specific outsourced Positively Healthcare related operation. Positively Healthcare will take no responsibility for cancellations or defaults by a third party supplier.
14) The decision of Positively Healthcare is final and legally binding. No correspondence will be entered into. By applying, applicants agree to and are bound by these general terms and all other special conditions that are specific to this offer.
